Do you suspect unprofessional misconduct of a licensee?
IF YOU SUSPECT CRIMINAL ACTIVITY, PLEASE REPORT TO LOCAL LAW ENFORCEMENT. Reports to OPLC of professional misconduct may result in disciplinary action against the license of a professional, but OPLC does not prosecute criminal matters.
Please review the list of requirements for a complaint below. The Enforcement Division may reject a complaint if we cannot identify the licensee to conduct an investigation. If your complaint is rejected, our office will contact you and request that you refile the complaint with full information.
REQUIREMENTS
A clear and concise statement of the facts including:
- WHO: The name of the individual that you claim committed professional misconduct
- WHAT: Identify the profession of the licensed person
- WHEN: The date and time the conduct occurred
- WHERE: The location where the conduct occurred
- HOW: A description of what happened including the names and contact information of any other witnesses
Your contact information, including:
- WHO: Your first and last name and date of birth
- WHERE: Your email address, mailing address, and telephone number
- OTHER: Please state whether you have attempted to resolve the complaint with the licensee, whether you have retained an attorney in the matter, and whether you have reported the conduct to any other agencies.
