Mental Health Survey

New Hampshire Mental Health Survey Now Open

NH Mental Health Practitioner Licensure Survey, RSA 330:A:10-a

The NH Division of Public Health Services, in partnership with the NH Board of Mental Health Practice, is collecting key practice and capacity data from licensed social workers, mental health counselors, pastoral psychotherapists, and marriage and family therapists during the license renewal cycle. The survey is a component of the license renewal process.

The data collected will be confidential and protected; the aggregated, de-identified data will only be used for statewide health care workforce assessment and health care access planning purposes.

The survey will take you about 3-15 minutes to complete.

For mental health practitioners due to renew between 7/1/2019 and 6/30/2020, to begin the renewal process, please complete the 2019/20 NH Mental Health Practitioner Survey before submitting your renewal materials.

For mental health practitioners due to renew between 7/1/2020 and 6/30/2021, to begin the renewal process, please complete the 2020/21 NH Mental Health Practitioner Survey before submitting your renewal materials.
