You can get a roster by drafting a letter that you would like a roster with a $25 fee including an email address where you want it to go. If paying by check make the check out to Treasurer, state of NH, or fill out the Credit Card Form pdf file. We accept Visa or MasterCard only by fax, mail or in person. You can mail it to 121 S. fruit St, Ste 201 Concord NH 03301. The roster will include the license type, status, effective date, expiration date, first name, last name, middle initial, business name and business address. No other information is included.
