You need to download the Request for a Letter of Good Standing form pdf file. There is a 5.00 fee with this request. We accept Visa or MasterCard only by fax, mail or in person (Credit Card Form pdf file).  You can mail it to 121 S. Fruit St, Ste 201 Concord NH 03301. They are done daily and are processed within five business days upon receipt. If you pay by check, make the check out to Treasurer, State of NH.

All Changes need to be submitted on a request for name change amendment form pdf file. There is no fee associated with this change but if you would like a corrected certificate you must write this request on the bottom of the form and send in a 5.00 fee.  We accept Visa or MasterCard only by fax, mail or in person (Credit Card Form pdf file)  You can mail it to 121 S.
