Initial License
Application for Initial Licensure
Embalmer Apprentice
To apply for an initial license as an embalmer apprentice, please complete the following paper application: Application for Initial License: Embalmer Apprentice
To avoid administrative delays, please send the completed application with the following:
- Application fee ($55.00; Checks may be made payable to “Treasurer, State of New Hampshire.”).
- Copy of high school diploma, certificate, or degree.
- A criminal history records check conducted through the Department of Safety.
Embalmer
To apply for an initial license as an embalmer, please complete the following paper application:

To avoid administrative delays, please send the completed application with the following:
- Application fee ($110.00; Checks may be made payable to “Treasurer, State of New Hampshire.”).
- Practical examination fee ($150.00; Checks may be made payable to “Treasurer, State of New Hampshire.”).
- Official transcript from post-secondary institution.
- Official transcript from mortuary school.
- Verification of licensure from other jurisdictions (if appliable)
- Certified copy of birth certificate with raised seal.
- A criminal history records check conducted through the Department of Safety.
Notice Regarding Practical Examinations
- The board will provide notice and instructions regarding scheduling a practical examination once your application has been reviewed and determined to be complete.
Funeral Director
To apply for an initial license as a funeral director, please complete the following paper application:

To avoid administrative delays, please send the completed application with the following:
- Application fee ($300.00; Checks may be made payable to “Treasurer, State of New Hampshire.”).
- Written examination fee ($150.00; Checks may be made payable to “Treasurer, State of New Hampshire.”).
Notice Regarding Written Examinations
- The board will provide notice and instructions regarding scheduling a written examination once your application has been reviewed and determined to be complete.
Funeral Homes and Funeral Chapels
To apply for an initial license to operate a funeral home or funeral chapel, please complete the following paper application: Application for Initial License: Funeral Homes and Funeral Chapels
To avoid administrative delays, please send the completed application with the following:
- Inspection fee ($250.00; Checks may be made payable to “Treasurer, State of New Hampshire.”).
- Confirmation from secretary of state’s office that the entity applying for licensure is in good standing and authorized to do business in New Hampshire.
- A copy of the legal document that authorizes the individual signing the application that the individual is authorized to sign on the applicant’s behalf.
Notice Regarding Inspections
- The board approved inspector will contact funeral homes and chapels to schedule an inspection.
Crematories
To apply for an initial license to operate a crematory, or to change ownership, please complete the following paper application: Application for Initial License: Crematories
To avoid administrative delays, please send the completed application with the following:
- Application Fee ($100.00; Checks may be made payable to “Treasurer, State of New Hampshire.”).
- Confirmation from secretary of state’s office that the entity applying for licensure is in good standing and authorized to do business in New Hampshire.
- A copy of the legal document that authorizes the individual signing the application that the individual is authorized to sign on the applicant’s behalf.
- For each crematory operator, an unexpired certified crematory operatory certificate.
- For each crematory operator, a criminal history records check conducted through the Department of Safety.
- For each crematory owner, a criminal history records check conducted through the Department of Safety.
Notice Regarding Inspections
- The board approved inspector will contact crematories to schedule an inspection.

License Renewals
Apprentice License Renewals
To request an extension of your apprentice license, you may use the following paper application:
Application for License Renewal Apprentice Embalmers
To avoid administrative delays, please send the completed application with the renewal fee ($55.00; checks may be made payable to “Treasurer, State of New Hampshire.”)
Embalmer, Funeral Director & Embalmer License Renewals
For the biennium ending June 30, 2023, all embalmers and funeral directors & embalmers will be required to renew their license by completing the following paper application: Application for License Renewal Embalmers, Funeral Directors & Embalmers
To avoid administrative delays, please send the completed application with the following:
- Fee ($110.00 for embalmers, $300.00 for funeral directors & embalmers; checks may be made payable to “Treasurer, State of New Hampshire.”)
- Continuing education certificates
- Note, the Office has received all continuing education certificates for those who completed courses offered by the New Hampshire Funeral Directors Association. Please do not submit certificates from NHFDA courses. If you completed courses not offered by NHFDA, please submit your continuing education certificates, even if previously submitted.
Notice Regarding Continuing Education Requirements:
- As a reminder, every embalmer and funeral director shall complete 14 hours of approved continuing education credits every biennial licensing period, per Frl 403.03. If this is your first time renewing and you have had a license 12 months or less, you are only required to complete 7 hours of continuing education credit.
- Per Frl 403.04, a maximum of 10 hours may be received via Home Study or Internet courses.
Funeral Home and Funeral Chapel License Renewals
All funeral homes and funeral chapels are now required to renew their license by completing the following paper application: Application for License Renewal Funeral Homes & Funeral Chapels
To avoid administrative delays, please send the completed application with the following:
- Inspection Fee ($100.00 for funeral chapels, $200.00 for funeral homes. Checks may be made payable to “Treasurer, State of New Hampshire.”).
- Confirmation from Secretary of State’s office that the entity applying for licensure is in good standing and authorized to do business in New Hampshire.
- A copy of the legal document that authorizes the individual signing the application that the individual is authorized to sign on the applicant’s behalf.
Notice Regarding Inspections

- Confirmation from Secretary of State’s office that the entity applying for licensure is in good standing and authorized to do business in New Hampshire.
- A copy of the legal document that authorizes the individual signing the application that the individual is authorized to sign on the applicant’s behalf.

Provided the Office receives your completed application and documentation by the expiration date, your license will remain active until the Board acts on your application. Your license status will be displayed as “Active-Pending Renewal” on the license lookup feature of our website until processed.
