Initial License

There are two pathways to Physician licensure in New Hampshire:  1) Applying directly through the Federation of State Medical Boards (FSMB) or; 2) Seeking expedited licensure through the Interstate Medical Licensure Compact Commission (IMLCC).

Apply Directly to New Hampshire through the FSMB

Applicants seeking licensure in New Hampshire may apply directly to the Board by completing the following steps:  
1.     Uniform Application through FSMB: FSMB | Uniform Application
2.    Verification of Credentials through FCVS: FSMB | Federation Credentials Verification Service
3.    Submission of State Addendum: State Addendum 
4.    Submission of Supporting Documents:
             a.    Affidavit and Authorization for Release of Information 
             b.    Fee of $328 (Check may be made payable to “Treasurer, State of New Hampshire”)
             c.    FBI Criminal History Check: How to Obtain a Criminal Background Check
             d.    Letter(s) of Reference
             e.    Recent 2x3 Photograph (may be submitted with Affidavit)

To expedite processing, please submit the State Addendum, all supporting documentation, and payment for processing fees in the same submission, along with the following checklist, directly to OPLC: Application Checklist: Physician License.  Per RSA 541-A:29, failure to provide all requisite information and documentation to OPLC within sixty (60) days of the date of your application may result in your application being denied.

Apply through the Interstate Medical Licensure Compact Commission

Applicants who are licensed in another jurisdiction, and whose state of primary licensure is a member of the Physician License | Interstate Medical Licensure Compact, may apply for expedited licensure through the Interstate Medical Licensure Compact Commission.  To do so, please complete all steps on the following checklist: Application Checklist: Expedited Physician License through Interstate Medical Licensure Compact Commission (IMLCC)

Renewal of License

Physicians - To complete the health professions survey requirement for license renewal, please visit the Health Professions Survey page. Note: Administrative licenses and inactive status licenses are exempt from the survey requirement.
